Strengths-Based Case Management services encourage recovery efforts for participants who are managing ongoing mental health conditions by identifying and coordinating a range of resources to help people achieve their goals. In-person services are primarily delivered in the field at the person’s preferred location however, participants may be seen in the office on occasion.

Services:

• Symptom management
• Life skills training
• Assistance with basic needs including food, clothing, shelter, and medical care  
• Educational and vocational planning 
• Connection to other support as needed (e.g. benefit access, legal- and community-based spiritual supports, etc.)
