County-Operated Strengths-Based Case Management Services
Strengths-Based Case Management (SBCM) services support recovery efforts for participants who are managing ongoing mental health conditions by identifying and coordinating a range of resources to help people achieve their goals. In-person services are primarily delivered in the field at the person's preferred location however, participants may be seen in the office on occasion. There are two County-Operated SBCM programs located in North Central and East Regions. *As clinically appropriate, clients of these programs will transition to Telecare AgeWise program upon turning 60 years of age.
