Community-Based Care

Community-based care is provided in facilities that offer 24/7 care and supervision for people with various levels of clinical and social needs to support recovery and rehabilitation within the community. See below to learn more about community-based care programs.

Assisted Living and Board & Care Facilities

Assisted living facilities, which include licensed board and care homes, provide 24/7 support and supervision for people who require assistance and cannot live independently. Within these settings, residents receive enhanced, recovery-focused support to help residents build on their strengths, manage symptoms, and achieve greater self-sufficiency.

There are two types of licensed assisted living/board and care facilities: Adult Residential Facilities (ARFs) and Residential Care Facilities for the Elderly (RCFEs). Both provide a home-like environment where residents receive care, supervision, and rehabilitative services, with eligibility for Medi-Cal being a key factor for admission.

Recuperative Care

Coming in 2025

Recuperative care provides adults with behavioral health conditions who are experiencing homelessness a safe place to be discharged following hospitalization, when they would otherwise be transitioning in and out of expensive and unnecessary hospital care.
