U.S. Department of Health and Human Services
The Health Information Privacy portal contains information and videos about individuals’ rights with respect to their protected health information (PHI). It also contains information for those entities covered by the Health Insurance Portability and Accountability Act (HIPAA).
The Office for Civil Rights handles complaints
related to HIPAA and is where you may file a complaint regarding your
PHI.
The Office for Human Research Protection publishes a variety of policy and regulatory guidance materials to assist in conducting ethical research that is in compliance with the federal regulations.
California Department of Health Care Services
The Office of Legal Services and Data Privacy Unit work collaboratively to implement the regulations of HIPAA and work with the County of San Diego to safeguard PHI and investigate privacy breaches and complaints.
State of California Department of Justice
The Office of the Attorney General's Privacy Unit enforces state and federal privacy laws, empowers Californians with regards to their rights and strategies for protecting their privacy, and provides information on topics like identity theft and online privacy.

Privacy Breaches & Identity Theft
ACO addresses privacy questions, privacy complaints, and reports of potential privacy breaches as they relate to HHSA programs and those of HHSA contractors. While we will do our best to assist you with any privacy question, we cannot address these topics as they relate to private businesses or other agencies. If you believe your privacy has been breached or have a question about your protected information, you may reach out to the agency in question or to a variety of State and Federal agencies who provide oversight.
Furthermore, if you are concerned about your protected information, you may want to take some proactive steps to help ensure your information is safe. The State of California provides an Identity Theft Victim Checklist for individuals who believe they are a victim of identity theft.
