CMS Pharmacy Information

The CMS Program covers prescribed medications for all products listed on the CMS formulary. All prescriptions must be filled by a pharmacy serving CMS Program or by your primary care provider. This formulary is modified and updated on a periodic basis. Directions for obtaining non-formulary prescriptions are detailed in the instructions section of the CMS formulary.
