Skilled Nursing and Care in a Secured Setting

Skilled nursing and secured care, also known as subacute services, provide support for people who are stepping down from psychiatric acute inpatient care or for those whose acuity may have intensified and need a higher level of care. See below to learn more about skilled nursing and secured care services.

Mental Health Rehabilitation Centers

Mental Health Rehabilitation Centers (MHRCs) are locked facilities that provide intensive 24/7 long-term support and rehabilitative services for adults with serious mental illness who would otherwise be placed in a state hospital or another mental health facility. The goal of MHRCs is to stabilize the person and prepare them for a successful transition into the community or to a less intensive level of care.

Skilled Nursing Facilities

Skilled Nursing Facilities (SNFs) offer 24/7 specialized care for people with serious mental illness in a supportive environment. Trained healthcare professionals help residents manage their mental health conditions with personalized treatment plans which may include medication management, therapy sessions, and assistance with daily activities such as bathing, dressing, and eating. The County of San Diego oversees Edgemoor, a distinct part skilled nursing facility operated by the County, and additonal contracted SNF programs that are listed in the table below.
