Older Adult Health

The Older Adult Health Program collaborates with County departments and community partners to assess needs, identify strategies and implement programs to support all people to age how and where they choose. The Older Adult Health Program works closely with San Diego County Aging and Independence Services to achieve San Diego’s  Age Well  Vision, which encompasses San Diego’s Age-Friendly and Dementia-Friendly Community initiatives, and strives for countywide Age-Friendly Health System  transformation in which every older adult receives evidence-based care geared towards what matters most to the older adult and their family.
