Mission Statement

To utilize the special skill set of CCS families, in collaboration with CCS staff, to address areas that need improvement, enhance areas that are working well, and optimize medical case management for clients and families.

What is FACT

Family Advisory Committee Team ( FACT) was created to provide an avenue for families to give feedback to CCS through open discussion.  FACT also allows families to gain greater knowledge of resources available to them thru the efforts of CCS, government entities, and other community partners.

Parents/caregivers who participate in FACT are encouraged to share their stories and in turn empower others to tell theirs.  FACT members bring their own ideas of how to improve CCS services, and the FACT meeting provides a forum to raise these up.  FACT also reviews CCS processes and procedures to help ensure that they are culturally sensitive and family centered. CCS recognizes parents and young adults are the experts of their own family needs and when armed with information, they are empowered to be more self-reliant.
