Meals on Wheels of San Diego County

Description:

  • Meals on Wheels is a program that delivers meals to individuals at home who are unable to purchase or prepare their own meals

Mama’s Kitchen

Description:

  • Mama's Kitchen offers a home delivered meal program for individuals that have been diagnosed with a critical illness and are unable to independently prepare their own meals

  • A referral form must be completed by a medical case manager, medical social worker or provider (MD, NP, PA, RD, RN or LCSW)
