All passport applicants applying at this facility must complete form DS-11.
You may be eligible to renew your passport by mail using the DS-82 form. However, please note, we do not process form DS-82 at this facility. This form must be mailed directly to the U.S. Department of State using the address listed on the application.
DS-11 Application for a U.S. Passport
This form is used to apply for a U.S. passport book and/or card in person at an acceptance facility.
DS-64 Reporting Your Lost or Stolen Passport
To replace your lost or stolen passport, you must apply in person and submit this form along with form DS-11.
DS-3053 Statement of Consent for Issuing a Passport to a Child
Complete this form if one parent or guardian cannot go with the child to apply in person for the child's passport. Submit this form in addition to the child's application (DS-11).
DS-5525 Statement of Special Family Circumstances
Complete this form if one parent or guardian cannot locate the other parent or guardian. Submit this form in addition to the child's application (DS-11).

Complete this form if you meet the requirements to renew your passport by mail.
DS-5504 Data Corrections, Some Name Change, and Limited-Validity Passports
Use this form if you need to change or correct your passport, or you need to replace a passport that was limited to less than 10 years validity (normal validity for an adult's passport).
DS-86 Statement of Non-Receipt of a U.S. Passport
Submit this form only if the U.S. Department of State has completed and mailed your passport and you did not receive it within 10 days of mailing. Call the National Passport Information Center at 1-877-487-2778 for assistance with this form.
