Loss of a Dependent

The Employee Benefits staff would like to offer support to you during this difficult time by providing assistance with your benefits.  We understand that the loss of a loved one can be a very difficult time. We will make it a priority to assist you with your health and life benefits and extend our deepest condolences over the passing of your loved one.

Life Insurance Claim

All regular employees have County paid Dependent Life Insurance in the amount of $2,000. If you covered a dependent under a Voluntary Accidental Death and Dismemberment plan, you may have an additional claim. In order to initiate the claim process, please call the Employee Benefits Division at 888-550-2203. We will initiate the claim for you and explain the next steps to expect in the process.

Critical Illness Claim

If you covered your dependent under the Allstate Critical Illness plan, and haven’t already made a claim, you may be eligible to make a claim now, depending on the nature of your dependent’s passing. The Critical Illness Plan pays a cash benefit to you for any of a comprehensive list of serious illnesses, including cancer, heart attack, stroke and more. Making Changes

If your dependent was on your health benefits, we can assist you with removing them from your active plans. Please provide a copy of the death certificate to begin the process, changes will be effective the 1st of the month after the dependent’s passing.

Consider Changing Your Tax Withholdings

You may wish to consult a financial counselor to discuss your situation. If you decide to change your tax withholdings, you can do so via Employee Self Service.
