USA DHMO Plan
The DHMO plan allows you to receive comprehensive coverage at set prices, called copays and is only available in California.
Dentists/Other Dental Care Specialists: You can only use dentists who participate in the DeltaCare DHMO network. You are required to designate a Primary Care Dentist who you will be assigned to for care. Dentists who participate in the DeltaCare DHMO network are called in-network dentists. There is no coverage if you go to out-of-network dentists.
Annual Deductible: You don’t need to pay an annual deductible before the plan begins to pay for a portion of covered dental services.
Copays: When you receive dental care, you pay a set dollar amount called a copay.
Orthodontia Care: You MUST use a provider in the DeltaCare Orthodontic Network. Confirm your orthodontist is in the Delta Care Orthodontic Network by calling DeltaCare at 844-697-0579 BEFORE you start treatment. If you’ve already started your orthodontic treatment under a previous plan, you may be able to continue that coverage when you switch to a DeltaCare USA plan. Designation of Primary Care Dentist
Newly enrolled employees can designate a Primary Care Dentist or facility by: scheduling an appointment with an in-network Primary Care Dentist or facility, registering for an online account, or calling DeltaCare USA at (844) 697-0579.
