Critical Illness Insurance

Critical Illness coverage through Lincoln Financial Group provides a lump-sum payout if you or an eligible dependent is diagnosed with a covered illness.  

You may cover yourself, your spouse/domestic partner and children under the age of 26.  In order to elect the benefit for your dependents, you must also be enrolled.

  • You can elect amounts of $10,000, $20,000, $30,000, or $40,000.
  • Spouses and child(ren) are covered at 50% of the benefit amount you elect.

Health Assessment Screening Benefit

Lincoln Financial Group pays an annual benefit for completing a health screening. Upon completion, you and your eligible dependents receive a $100 cash benefit.  Call Lincoln Financial Group to report your wellness exam.

  • Follow the prompts to submit your Health Assessment claim telephonically. You will need the following information: 
    • Your SSN or work ID 
    • Group Policy Number: CI-0000630053 
    • Patient’s name and birthdate 
    • Tests/screenings performed 
    • Payment preference
